SUBJECT: Front Fog Lights |
Summary:On certain mini vans equipped with front fog lights, the fog
light bulb socket may have been installed improperly.
If the vehicle continues to be used with this condition,
the bulb socket and the wiring harness may separate from the fog light housing
if exposed to shock and/or vibration and drop inside the bumper. Should this
occur, the operation of the fog light may then cause the bumper to burn.
Dealers will inspect the installation condition of the fog
light bulb assembly, and repair it when necessary. Owner notification began
March 31, 2003. Owners who take their vehicles to an authorized dealer on an
agreed upon service date and do not receive the free remedy within a reasonable
time should contact Mazda at 1-800-222-5500.

Dear Mazda Dealer:
Mazda Motor Corporation has decided that a defect, which relates to motor
vehicle safety, exists in certain 2002 and 2003 model year Mazda MPV vehicles
produced from November 26, 2001 through January 21, 2003.
On certain 2002 and 2003 MPV vehicles, it is possible that the front fog
lights may have been improperly installed. If the vehicle continues to be
driven, normal driving conditions could cause the bulb socket and wiring
harness to become dislodged from the fog light housing and drop inside the
front bumper. Should this occur, continual operation of the vehicle’s fog
lights could cause a fire in the front bumper.
Mazda has decided to conduct a recall campaign to inspect the front fog
light and to replace it with a new one if the upside-down installation of the
fog light bulb is found.
Owners will be notified by first class mail on March 31, 2003. Detailed
information regarding this recall campaign is provided in Attachment I. The
inspection and repair procedure is provided in Attachment II. The owner
notification letter is provided in Attachment III.
The National Traffic and Motor Vehicle Safety Act prohibits selling or
leasing any subject vehicles without performing the necessary inspection
and/or repair for defects or failures. Failure to perform applicable recalls
before sale can result in extensive fines and penalties from the Federal
Government. Therefore, you must complete this recall for all affected vehicles
in your inventory prior to sale or lease.
Please make certain that the appropriate personnel in your dealership are
familiar with the details of this recall campaign before responding to

ATTACHMENT II
A. VEHICLE INSPECTION PROCEDURE
- 1. Verify that the vehicle is within the following range:
- VIN Range:
- 2002 Model Year - JM3 LW28J*20 300020 - 334294
- 2003 Model Year - JM3 LW28J*30 334903 - 359396
- If the vehicle is within the above range, proceed to step 2.
- If the vehicle is not within the above range, return the vehicle to the
customer or inventory.
- Perform a Warranty Vehicle Inquiry using your eMDCS System and inspect
vehicle for an Authorized Modification Label RECALL 0903C attached to
the underside of the vehicle’s hood. Refer to illustration below.
NOTE: Be sure to verify RECALL number as the vehicle may have multiple
RECALL labels.

- If eMDCS displays RECALL 0903C OPEN and an Authorized
Modification Label is not present, the RECALL has not been performed.
Proceed to “B. CORRECTION PROCEDURE, FRONT FOG LIGHTS INSPECTION / REPAIR
PROCEDURE”.
- If eMDCS displays RECALL 0903C OPEN and an Authorized
Modification Label is present, contact the Warranty Department at
(800) 662-6779. They will update the vehicle warranty.
- If eMDCS displays RECALL 0903C CLOSED and an Authorized
Modification Label is not present, proceed to “C. AUTHORIZED
MODIFICATION LABEL INSTALLATION”.
- If eMDCS displays RECALL 0903C CLOSED and an Authorized
Modification Label is present, the RECALL has already been completed.
Return the vehicle to the customer or inventory.
- If eMDCS does not display RECALL 0903C OPEN or RECALL 0903C
CLOSED, this RECALL does not apply to the vehicle. Return vehicle to the
customer or inventory.
B. CORRECTION PROCEDURE - FRONT FOG LIGHTS INSPECTION / REPAIR PROCEDURE
WARNING: When a halogen bulb breaks, it is dangerous. These bulbs
contain pressurized gas. If one is broken, it will explode and serious
injuries could be caused by the flying glass. If the glass portion is touched
with bare hands, body oil could cause the bulb to overheat and explode when
lit. Never touch the glass portion of the bulb with your bare hands and always
wear eye protection when handling or working around halogen bulbs.
- Place the vehicle on a lift and fully turn the steering wheel to one side
to gain access to one of the fog lights.
- Remove four fasteners and four screws on the inner fender splash shield.

- Disconnect the harness connector and remove the fog light bulb socket by
turning the bulb socket counter clockwise and pull the bulb socket out.
CAUTION: Grasp the bulb socket only by its plastic base and do not
touch the glass. The oil from your hands could cause the bulb to break the
next time the fog lights are operated.

- Holding the bulb socket with the connector pointing downward, compare the
bulb socket to the illustration below:
- If the bulb socket is GOOD, proceed to step 6.
- If the bulb socket is BAD, proceed to step 5.

- Remove the entire fog light assembly by removing three screws, then
install a new assembly (the bulb socket comes with the assembly).

- Re-install the fog light bulb socket.
CAUTION: Do not allow the waterproof cover to be pinched, turn the fog
light bulb socket clockwise until a click is heard and the connector is
pointing downward at the 6 o’clock position

- Re-connect the fog light bulb socket harness connector.
- Confirm that the fog light is operative.
- Re-install the splash shield, then begin checking the other fog light.
- If fog light assembly has been replaced, perform the fog light adjustment
according to Workshop Manual (section 09-18-7 FRONT FOG LIGHT ADJUSTMENT).
C. AUTHORIZED MODIFICATION LABEL INSTALLATION
Complete an “Authorized Modification Label” with the RECALL number written on
the sticker and affix it to the underside of the vehicle’s hood. Refer back to
the illustration under “A. VEHICLE INSPECTION PROCEDURE”.

This notice is sent to you in accordance with the requirements of the
National Traffic and Motor Vehicle Safety Act.
Mazda Motor Corporation has decided that a defect, which relates to motor
vehicle safety, exists in certain 2002 and 2003 model year Mazda MPV vehicles
produced from November 26, 2001 through January 21, 2003. If you are a
recipient of this notice, your vehicle is included in this campaign.
What is the problem?
On certain 2002 and 2003 MPV vehicles, it is possible that the front fog
lights may have been improperly installed. If the vehicle continues to be
driven, normal driving conditions could cause the bulb socket and wiring harness
to become dislodged from the fog light housing and drop inside the front bumper.
Should this occur, continual operation of the vehicle’s fog lights could cause a
fire in the front bumper.
What will Mazda do?
Your Mazda dealer will inspect the installation condition of the fog light
bulb assembly, and repair it when necessary free of charge. This
inspection and/or repair should take approximately one (1) hour to complete,
however, it may take longer depending on the service workload at your Mazda
dealership.
What should you do?
As Mazda is concerned about your safety, we would encourage you to make an
appointment with any authorized Mazda dealer to have the front fog light
inspected and replaced when necessary. You do not need to bring this notice to
the dealer, but it may assist in the check-in process.
What if you have already paid for replacement of the front fog light?
If you have already paid for the inspection/repair or replacement of the
front fog light(s) due to the separation or inappropriate installation of the
fog light(s) bulb socket and any associated vehicle damage resulting from this
problem, you may be eligible for reimbursement of reasonable repair expenses
based on Mazda’s repair standards. Please complete the enclosed “Reimbursement
Application Form”, including the necessary documentation and mail it to us in
the pre-addressed envelope provided, allowing 6-8 weeks for processing.

Moved or no longer own this vehicle?
If you have moved or no longer own your Mazda MPV, please complete the

If you are the lessor of a subject Mazda MPV, we strongly encourage you to
forward this recall notification to the lessee, as it is critical that the
current driver of the vehicle is notified of the recall program.

- Description of the concern reported
- Inspection/Repair of the front fog light(s) due to improper
installation or separation

without the legible copy of the paid repair order or invoice will be returned
