Mazda Motor Corporation has decided to conduct a Special Service Program to
replace the fan control module on 2002-2003 MPV vehicles produced from November
26, 2001 through July 1, 2003.
It is possible the soldering strength on a terminal in the fan control module
may be insufficient, causing a crack during use. If this failure occurs, the
cooling fans may stop operating, causing the coolant temperature to rise, or the
cooling fans may continue operating and may drain the vehicle's battery. Owners
of affected vehicles will be notified by first class mail beginning August 30,
2004. This SSP will be included with the mailing of Recall 2204H.

PARTS ORDERING
Dealers will automatically receive a quantity of Fan Control Module Kits on
stock orders. Parts shipments will begin August 23, 2004 and will arrive on
dealers’ next stock order. Dealers will receive Fan Control Module Kits by
August 30, 2004.
If dealers need additional parts after receiving the initial parts shipment,
order them by faxing an IPFAX order form to Mazda Corporate Dealer Assistance
Group at (866) 366-2061. Up to 5 additional Fan Control Module Kits may be
requested with each IPFAX order form processed. The order form can be found:

A rental car may be provided to the customer if eligible based on the terms
and conditions of the Rental Car Reimbursement Program, policy 12.0. Rental car
reimbursements are available only on 2001 and newer vehicles within the mileage
and time limitations under the New Vehicle Limited Warranty. If the customer was
placed in a rental car while the campaign was being completed, submit a separate
claim/problem using the standard rental claim information.

ATTACHMENT II - INSPECTION AND REPAIR PROCEDURES
A. VEHICLE INSPECTION PROCEDURE
- Verify that the vehicle is within the following range:
- 2002 - 2003 MPV vehicles built from November 26, 2001 through July
1, 2003
VIN Range: JM3LW28**2*300020 - JM3LW28**3*374773
- If the vehicle is within the above range, proceed to Step 2.
- If the vehicle is not within the above range, return the vehicle
to the customer or inventory.
- Perform a Warranty Vehicle Inquiry using your eMDCS System and inspect
vehicle for an Authorized Modification Label SSP60 attached to the
vehicle’s hood. Refer to eMDCS System - Warranty Vehicle Inquiry Results
table below.
NOTE: Be sure to verify SSP number as the vehicle may have multiple
SSP labels.

B. REPAIR PROCEDURE
- Record customer’s pre-set radio stations.
- Disconnect and remove the battery to gain access to the Fan Control
Module.
- Remove the two (2) Fan Control Module bolts; discard the bolts.

- Disconnect the three (3) electrical connectors from the Fan Control
Module.
- Attach the three (3) connectors to the new Fan Control Module.
- Secure the module with two (2) new bolts provided in the kit.
Tightening Torque: 26-34 in·lbf (29-40 kgf·cm)
- Reinstall battery and connect the cables.
- Re-enter customer’s radio pre-set stations.
- Proceed to “C. AUTHORIZED MODIFICATION LABEL INSTALLATION”.
C. AUTHORIZED MODIFICATION LABEL INSTALLATION
Complete an “Authorized Modification Label” with the Recall number written on
the sticker and affix it to the vehicle’s hood. Refer back to the illustration
under “A. VEHICLE INSPECTION PROCEDURE”.

ATTACHMENT III – OWNER NOTIFICATION LETTER

2002-2003 MPV Fan Control Module
Special Service Program (SSP) 60
Dear Mazda Owner:
Mazda Motor Corporation has decided to conduct a Special Service Program to
replace the fan control module on 2002-2003 MPV vehicles produced from November
26, 2001 through July 1, 2003. If you are the recipient of this notice, your
vehicle is included in this program.
It is possible the soldering on a fan control module terminal may be
insufficient, causing a crack during use. If this failure occurs, the cooling
fans may stop operating, causing the coolant temperature to rise, or the cooling
fans may continue operating and may drain the vehicle’s battery.
Mazda will replace the fan control module with a modified one free of
charge. The repair may take approximately thirty minutes to complete
depending on the service workload at your Mazda dealership.
As a reminder, your Mazda Driver’s Assurance Plan provides alternate
transportation any time an owner’s vehicle is at an authorized Mazda dealership
for a warranty repair. To be eligible for alternate transportation, your vehicle
must be within the mileage and time limitations under the Mazda New Vehicle
Limited Warranty and adhere to the Rental Car Reimbursement policy. Ask your
dealer for details about the Mazda Rental Car Reimbursement Program.
If you have already paid for the inspection/replacement of the fan control
module, you may be eligible for reimbursement of reasonable repair expenses
based on Mazda’s repair standards. Please complete the enclosed “Reimbursement
Application Form,” include the necessary documentation, and mail it to us in the
pre-addressed envelope provided, allowing 6-8 weeks for processing.

consult your local yellow pages.
If you have moved or no longer own your Mazda vehicle, please complete the
enclosed prepaid Information Change Card as soon as possible. This
enables us to update our records and notify the current owner. Federal law
requires that any vehicle lessor receiving this notice must forward a copy of
this notice to the lessee within ten days.
