SUBJECT: Passenger air bag may not deploy in a collision
Summary:
On certain passenger vehicles, some of the passenger air bag modules were not properly welded and may not deploy in a collision.

The seat occupant may not be properly protected in the event of a collision, increasing the risk of personal injury.

Dealers will replace the passenger air bag module. Owner notification is expected to begin during August 2002. Owners who take their vehicles to an authorized dealer on an agreed upon service date and do not receive the free remedy within a reasonable time should contact Mazda at 1-800-222-5500.

2001 MPV – Passenger-Side Airbag Module

Dear Mazda Dealer:

Mazda Motor Corporation has determined that a defect, which relates to motor vehicle safety, exists in certain 2001 model year Mazda MPV vehicles produced from December 21, 2000 through December 25, 2000.

In some 2001 MPV vehicles, it is possible that the passenger-side airbag module may not deploy properly during a frontal impact. On the passenger-side airbag, the diffuser may come off the inflator during airbag deployment due to improper welding. In that case, expanded gas cannot be supplied to the bag and the airbag module may not deploy in a frontal impact, resulting in an increased risk of injuries.

Mazda has decided to conduct a recall campaign to replace the passenger-side airbag module with a modified airbag module.

There are twenty-five subject vehicles and Owners will be notified by first class mail on October 4, 2002. Detailed information regarding this recall campaign is provided in Attachment I. The repair procedure is provided in Attachment II. The owner notification letter is provided in Attachment III.

The National Traffic and Motor Vehicle Safety Act prohibits selling or leasing any subject vehicles without performing the necessary repair for defects or failures. Failure to perform applicable recalls before sale can result in extensive fines and penalties from the Federal Government. Therefore, you must complete this recall for all affected vehicles in your inventory prior to sale or lease.

Campaign labels are supplied at no charge and are available in 8 ½” X 11” sheets containing 18 labels. Please use MStore for ordering additional labels.

IMPORTANT NOTES:

  1. DO NOT DEPLOY THE AIRBAG MODULE ONCE IT IS REMOVED FROM THE VEHICLE. THE UNDEPLOYED AIRBAG MODULE MUST BE SHIPPED DIRECTLY TO THE SUPPLIER, NOT TO MAZDA. CONTACT THE SUPPLIER AT 1-800-822-1543 FOR SHIPPING AND PACKAGING REQUIREMENTS.

Attachment II
  1. VEHICLE INSPECTION PROCEDURE
    1. Verify that the vehicle is within the following range:
      • VIN Range: JM3 LW28**10 186689 - 186994
        • If the vehicle is within the above range, proceed to step 2.
        • If the vehicle is not within the above range, return the vehicle to the customer or inventory.
    2. Perform a Warranty Vehicle Inquiry using your MDCS System and inspect vehicle for an Authorized Modification Label RECALL 0802H attached to the underside of the vehicle’s hood. Refer to illustration below.
      NOTE: Be sure to verify RECALL number as the vehicle may have multiple RECALLs.
      • If MDCS displays RECALL 0802H OPEN and an Authorized Modification Label is not present, the RECALL has not been performed. Proceed to “B. CORRECTION PROCEDURE, PASSENGER-SIDE AIR BAG MODULE REPLACEMENT”.
      • If MDCS displays RECALL 0802H OPEN and an Authorized Modification Label is present, contact the Warranty Department at (800) 662-6779. They will update the vehicle warranty.
      • If MDCS displays RECALL 0802H CLOSED and an Authorized Modification Label is not present, proceed to “C. AUTHORIZED MODIFICATION LABEL INSTALLATION”.
      • If MDCS displays RECALL 0802H CLOSED and an Authorized Modification Label is present, the RECALL has already been completed. Return the vehicle to the customer or inventory.
      • If MDCS does not display RECALL 0802H OPEN or RECALL 0802H CLOSED, this RECALL does not apply to the vehicle. Return vehicle to the customer or inventory.
         
  2. CORRECTION PROCEDURE
    PASSENGER-SIDE AIR BAG MODULE REPLACEMENT
    WARNING:
    • A live (undeployed) air bag module may accidentally deploy when it is handled and cause serious injury. When carrying a live (undeployed) air bag module, point the front surface away from your body to lessen the chance of injury in case it deploys.
    • A live (undeployed) air bag module placed face down on a surface is dangerous. If the air bag module deploys, the motion of the module can cause serious injury. Always face the front surface up to reduce the motion of the module in case it accidentally deploys.
    • Never use air powered tools to remove the air bag module.
    1. Record the radio station pre-sets.
    2. Turn the ignition switch to LOCK position.
    3. Disconnect the battery negative cable and wait for more than 1 minute.
    4. Remove the glove compartment.
    5. Disconnect the “yellow” air bag module electrical connector.
    6. Remove the two retaining bolts holding the air bag module to the dash board frame.
    7. Carefully lift the air bag module from the dash. Return the undeployed module to your Parts Manager. Have your Parts Manager call ( 1-800-822-1543 ) for return shipping instructions and a shipping container.
      WARNING: Do not disassemble the air bag module or deploy it.
      NOTE: DO NOT SHIP AIRBAG MODULE TO MAZDA, CALL TOLL FREE NUMBER (ABOVE) FOR INSTRUCTIONS.

      The manufacturer of the air bag module will provide the following:
      • Department Of Transportation (DOT) / United Nations (UN)-specification packaging
      • Pre-printed DOT-compliant shipping documentation
      • DOT-required Hazmat training
      All of the above are for the purposes of this one-time shipment only. The training provided by the air bag module manufacturer is not comprehensive, and is not accepted by MNAO for any hazardous materials shipping to MNAO locations. For MNAO-approved Hazmat training, contact your District Customer Support Manager for details.
       
    8. Install the new air bag module into the dash and tighten the two retaining bolts.
      Tightening Torque: 6.87 - 9.8 N·m (70 - 100 kgf·cm, 60.8 - 86.7 in·lbf)
      NOTE: Make sure top of air bag is flush with dash at all four corners.
    9. Re-connect the “yellow” electrical connector and attach it as shown.
    10. Re-install the glove compartment.
    11. Re-connect the battery negative cable.
    12. Turn the ignition switch to the ON position.
    13. Verify that the Air Bag System warning light illuminates for approximately 6 seconds and then goes off.
      • If the Air Bag System warning light does not operate in the manner described above, there are malfunctions in the system. Inspect the system using the on-board diagnostic according to Workshop Manual (section 08-02 FOREWORD).
    14. Re-set the radio station pre-sets.
       
  3. AUTHORIZED MODIFICATION LABEL INSTALLATION
    Complete an “Authorized Modification Label” with the RECALL number written on the sticker and affix it to the underside of the vehicle’s hood. Refer back to the illustration under “A. VEHICLE INSPECTION PROCEDURE”.

Dear Mazda Owner:

This notice is sent to you in accordance with the requirements of the National Traffic and Motor Vehicle Safety Act.

Mazda Motor Corporation has determined that a defect, which relates to motor vehicle safety, exists in certain 2001 model year Mazda MPV vehicles produced from December 21, 2000 through December 25, 2000. If you are a recipient of this notice, your vehicle is included in this campaign.

What is the problem?
In some 2001 MPV vehicles, it is possible that the passenger-side airbag module may not deploy properly during a frontal impact. On the passenger-side airbag, the diffuser may come off the inflator during airbag deployment due to improper welding. In that case, expanded gas cannot be supplied to the bag and the airbag module may not deploy in a frontal impact, resulting in an increased risk of injuries.

What will Mazda do?
Your Mazda dealer will replace the passenger-side airbag module with a defect-free airbag module free of charge. This repair should take approximately one (1) hour to complete, however, it may take longer depending on the service workload at your Mazda dealership.

What should you do?
As Mazda is concerned about your safety, we would encourage you to make an appointment with any authorized Mazda dealer to have the passenger-side airbag module replaced. You do not need to bring this notice to the dealer, but it may assist in the check-in process.

Moved or no longer own this vehicle?
If you have moved or no longer own your Mazda MPV, please complete the enclosed prepaid Information Change Card as soon as possible. This enables us to update our records and notify the current owner.

If you are the lessor of a subject Mazda MPV, we strongly encourage you to forward this recall notification to the lessee, as it is critical that the current driver of the vehicle is notified of the recall program.
